Introduction
The synthetic form of chemotherapeutic antibiotic is 'Ciprofloxacin' which belongs to the drug class fluoroquinolone. It is considered as the second generation of fluoroquinolone antibacterial. It interfere the action of enzyme that causes DNA to reverse after copying, which further stops the DNA and protein synthesis and finally kills the bacteria. There are more than three hundred diverse brand names of Ciprofloxacin being marketed throughout the world. It is being marketed as Baysip, Ciflox XL, Cipro XR, Ciloxan, Prociflor, Ciproxin, Cipro and the most recent one is Proguin in United States, United Kingdom and Canada. In Russia and India it is being marketed as Ciprex and in Spain as Cetraxal. Under various brand names the Ciprofloxacin is accessible as generic drug. While in veterinary medicine Ciprofloxacin has a limited use. Bayer A.G was the first who patented Ciprofloxacin in 1983 and then in 1987 it was approved by FDA (U.S. Food & Drug Administration). There are 12 FDA approved uses of Ciprofloxacin for humans. Ciprofloxacin also has its uses in veterinary. It also interacts with thyroid medication and natural and herbal supplementation.
Composition
The main component or ingredient for this drug is CIPROFLOXACIN HYDROCHLORIDE. The reported molecular weight of this substance is 385.8. It is yellowish in color. It comes in varying strength. Inactive components of this drug are titanium dioxide, magnesium stearate, cornstarch, and crospovidone.
Use
Several infections including diseases of joints and bones, gastroenteritis, endocarditis, cellulitis, infections of respiratory and urinary tracts, prostatischancroid and anthrax are being treated with Ciprofloxacin. It is not recommended as first line antibiotic for the infections of bacterial chronic prostitis, urinary and lower respiratory tracts. Although, it is recommended as the first line antibiotic to cure acute cystitis in females. Ciprofloxacin also suggested for different skin infections, infectious diarrhea and typhoid fever.
Over and above it can also be used in combination with certain other drugs as mixture with metronidazole is being used for intera abdominal complicated infections and also in combination with piperacillin is being used in empirical therapy for feverish neutropenic patients.
Dosage and administration
Ciprofloxacin extended release and immediate release pills are not the same. Ciprofloxacin extended release pills must be orally administered once daily as explain in the following table of Dosage Guidelines.
For Urinary Tract uncomplicated Infection the unit of dose 500 mg is recommended for 3 Days.
(Acute Cystitis)
For Urinary Tract complicated Infection the unit of dose 1000 mg is recommended for 7-14 Days
For acute Pyelonephritis Uncomplicated infection the unit of dose 1000 mg is recommended for 7-14 Days
The extended release Ciprofloxacin tablets must be administered not less than 2 hours or 6 hours later than antacids, sucralfate, VIDEX® chewable tablets or other extremely buffered drugs. While ciprofloxacin extended release pills can be taken with different meals including dairy products and milk. Ciprofloxacin extended release pills are supposed to be swallowed whole.
Contraindication
Ciprofloxacin pills are contraindicated in those persons having history of allergic reaction to Ciprofloxacin, antimicrobial agents from quinolone class, or any other product components. Concomitant direction with tizanidine is being contraindicated. Ciprofloxacin is also measured to be exist contraindicated in pediatric population, nursing mothers and during pregnancy.
Important Precautions
The function of patients hepatic and renal must be taken in consideration to avoid from any situation that may develop toxicity. Through the renal excretion Ciprofloxacin can be eliminated. While the digested or metabolized drug can be cleaned through kidneys. Therefore the toxicity may be higher in damaged renal functions. The time period for the treatment would be regarding the severity of infection and that is generally from 7-14 Days.
Side effects
The antibiotic fluoroquinolones may cause severe adverse effects as compared to the other antibiotics. These effects may be mild, moderate and occasionally severe. The U.S. Food & Drug Administration demanded new notice in 2004 to be supplementary to all fluoroquinolones including Ciprofloxacin concerning peripheral neuropathy, heart problems, tendon damage and muscle breakdown.
The ciprofloxacin therapy may cause the psychotic reactions, acute pancreatitis, confusing states, depression of bone marrow, hemolytic anemia and interstitial nephritis. The permanent as well as temporary side effects include vision loss, double irreversible vision chorea, abdominal pain and psychosis.
storage
Ciprofloxacin may be stored at room temperature (15-30°C) in any air tight container, keep away from heat, light and moisture. Ciprofloxacin should be placed away from the children and from pets.
